Prior to your appointment at Delfin Studios LLC, clients must have their temperature taken AT HOME and answer the questions listed below.

This guidance is intended for screening of clients and artist(s) prior to appointment. It is not intended for people with confirmed or suspected COVID-19, including persons under investigation. Individuals with confirmed or suspected COVID-19 should follow the guidance from their physician and CDC guidelines.

Do you have a temperature of 99.5 degrees or higher?
Have you had ANY of the following symptoms within the past 24 hours? Fever/chills, shortness of breath or difficulty breathing, fatigue, muscle or body aches, headaches, new loss of taste or smell, sore throat, congestion or runny nose, nausea or vomiting, diarrhea, or new or worsening cough?
Have you had ANY of the following symptoms within the past 24 hours?
In the two weeks did you come into contact with someone diagnosed with COVID-19?
In the past 2 weeks have you traveled internationally or domestically?
